Adult Health Check

The Adult Health Team is a multidisciplinary group delivering holistic, culturally safe, community-led care.

They support physical, mental, social, and emotional wellbeing through prevention, early intervention, treatment, and chronic disease management.

GYSHAC’s Adult Health Team are a group of clinical professionals who are of Aboriginal descent and non-Indigenous:

  • Registered Nurse
  • Endorsed Enrolled Nurse
  • Enrolled Nurse
  • Indigenous Health Worker
  • Trainee Indigenous Health Worker

The clinical team provide:

  • Culturally responsive and trauma-informed care that respects Aboriginal identity, values, and family structures
    Conduct adult health checks (715 health checks), routine screenings, and preventative care
  • Support clients living with chronic conditions such as diabetes, cardiovascular disease, kidney disease, asthma, and cancer
  • Develop care plans that involve family and community support networks
  • Coordinate access to allied health, specialists, and hospital services when required
  • Promote self-management and healthy lifestyle practices that are culturally relevant
  • Coordinate appointments, transport, referrals, and follow-ups for clients
  • Home visits and medication delivery
The clinical team also provides referrals to other GYHSAC services, including Care Coordination and Case Management, Family Healing, Family Wellbeing, Men’s and Women’s Programs, Social and Emotional Wellbeing (SEWB), as well as home visits and medication delivery.
