Integrated Team Care

The ITC program improves health outcomes by providing coordinated, client -centered care, supporting access to appropriate health services and helps clients manage chronic conditions more effectively.

The ITC Program supports clients with complex chronic conditions who need one-on-one, coordinated, multidisciplinary care. 

It aims to improve chronic disease management and access to appropriate healthcare through care coordination and supplementary services for eligible clients.

Chronic disease conditions, include but are not limited to:

  • Diabetes and related conditions
  • Cardiovascular disease
  • Chronic kidney disease
  • Cancer
  • Respiratory disease
  • Mental illness

Through the ITC Program, eligible clients may access support including:

  • Arranging specialist medical appointments
  • Medical Aids (conditions apply)
  • Dose administration aids
  • Assistive breathing equipment (including asthma spacers; masks for asthmas spacers and nebulisers; continuous positive airways pressure (CPAP) machines; accessories for CPAP machines
  • Blood sugar/glucose monitoring equipment
  • Medical footwear prescribed and fitted by a podiatrist
  • Mobility aids
  • Spectacles
  • Specialist Appointments

Most importantly, the ITC Program champions culturally safe, community-led care –  empowering our Yarrabah mob to take control of their health and live stronger, longer lives.
